We provide comprehensive care. Our highly responsive model allows seniors to receive care in the comfort of his or her own home or any setting. We also advocate for Medicare and other services to which they are entitled. Our programs are available 24 hours a day, 7 days a week, 365 days a year.
Our approach to care begins with a thorough assessment of a clients needs, including medical, functional, psychological, social and financial issues. Based on this assessment, nurse and social worker care managers develop a comprehensive care plan that is customized to meet the unique needs of each client. Our customized service program allows families to select just the right care for their specific needs and circumstances. This flexibility enables the overall cost to be affordable. Areas that distinguish SeniorBridge Comprehensive Care are:Dedicated Geriatric Care ManagersClients are assigned a Geriatric Care Manager (either a social worker or a nurse) who will help develop a plan that is customized to their needs, oversee all the care we provide and answer all questions.Disease Management and MonitoringSeniorBridge nurse care managers coordinate treatment of our clients diseases, including using in-home monitoring of a patient's clinical condition.At-Home CaregiversWe employ only those who are highly experienced. Our screening process includes credit, drug, criminal and medical screening. We hold extensive personal interviews, which include role-plays conducted by our Geriatric Care Managers. We select only individuals who we would be willing to trust in our home.Superior SupervisionEvery at-home caregiver is oriented, supervised and guided continuously by a Geriatric Care Manager.
